We offer individual, family and group counseling services. As a Medicare approved Clinical Social Worker, I can provide counseling services under the client's Medicare insurance.
Informed Decisions for Seniors offers a private pay wrap-around service where a social worker will conduct a life inventory assessment, research resources and provide the completed assessment form and individual findings to the client. How many times have we heard family members in emergency situations state, "Mom or dad never discussed their medical conditions with us, their finances, or their medical wishes?" All of this is reviewed and addressed within our unique wrap-around service. We help the client's voice be heard through this service so when a crisis occurs, affairs are in order and wishes known.
We also offer private pay hourly services such as assisting in finding appropriate placement, consulting and assisting in the Medicaid application process. Other services include crisis management, seminars and advocacy. Currently, I can provide complimentary seminars on topics such as Medicaid, Medicare, VA Benefits, Community Resources and Advance Directives.
Informed Decisions for Seniors can make house calls and provides personalized service for those in the Philadelphia and Bucks County area.
